Rules and Restrictions

Overview of UnitedHealthcare Community Rewards Program (the "Program").

The Program is a web-based application in which eligible members can earn points for completing health-related activities. The points can be accumulated and redeemed for merchandise rewards.

UnitedHealthcare Community Plan ("Company") reserves the right to add to and/or change the Program Terms & Conditions at any time. This means, for example, that we may change the number of points earned under the Program, or the number of points required to redeem rewards, or impose caps on earning and/or redeeming points, and/or cancel rewards. In addition, we reserve the right to terminate the Program by providing you with one (1) month prior notice. During the 1-month notice period, we may change or cancel some or all of the then-current rewards. Your right to earn points and redeem accumulated points will terminate within the month after you receive notice.

ELIGIBILITY

The Program is void where prohibited by law.

To register for a Program account ("Account"), you, ("Owner") must provide the member ("Participant") information and member ID number.(If you are the parent or guardian of a participant under the age of 17, you are also considered the “Owner” of the minor’s account). Once the Participant’s member ID is verified, Participants become eligible to earn points. Within the Account, any earned points will be held in a point account ("Point Account"). Owners can only create one Account. Multiple Participants can be added under an individual Account in situations where the Owner is the parent or guardian of the Participants. There can only be one instance of a Participant enrolled in the Program at any given time.

Ohio: The Participant(s) must be 17 years of age (or younger) or an adult age 18-64, and a member of UnitedHealthcare Community Plan in the state of Ohio (OH).

Wisconsin: The Participant(s) must be 17 years of age (or younger) or a Childless adult age 18-64, and a member of UnitedHealthcare Community Plan in the state of Wisconsin (WI).

Arizona: The Participant(s) must be a member of UnitedHealthcare Dual Complete in the state of Arizona (AZ).

Iowa: The Participant(s) must be a member of UnitedHealthcare Community Plan in the state of Iowa (IA).

If the Company determines a Participant is not eligible to participate in the Program, we reserve the right to remove them from the Account, and the Participant will lose any earned points. If no Participants within an Account are eligible, the entire Account will be closed and any earned points will be rescinded.
If the Participant is inactive in the Program for a period of one (1) year, the Company reserves the right to close the Account and rescind any points accumulated to-date.

COLLECTING POINTS

The Participant or parent/guardian of the Participant, as applicable, is responsible for registering and logging point activity.

In participating in the Program, points can be earned by logging into and participating in the online application. Points will be rewarded for healthy habits done by or on behalf of the Participant. This includes, but is not limited to the following activities: brushing teeth, washing hands, eating healthy, confirming their PCP, reading the member newsletter, completing the welcome call, doctor visits, immunizations and flu vaccines.

Points are not available for redemption until we verify that you have earned them (e.g., they may be marked "pending" points) and are not eligible for use for rewards until verified. Points accrued in the Point Account do not constitute property belonging to you.Although in some instances points may be rolled over for use in another year, Points are not transferable by operation of law or otherwise to any person or entity and cannot be transferred to any other Program account or for cash or form of remuneration.

The Program will comply with applicable State and Federal regulations related to annual incentive payment limits. An individual Participant will be restricted from earning points beyond the total equivalent dollar value annually.

By participating in the Program, you agree to only submit true and correct information. Should we discover that you have falsified any information, we reserve the right to rescind points that cannot be validated.

REDEEMING POINTS

Points earned during active membership may be redeemed at any time as long as the program is active. Should the program be discontinued, Participants will receive a notification sent to the email address associated with their account allowing them to redeem available points within 90 days.

If the Participant is not a member of UnitedHealthcare Community Plan or if his/her membership with UnitedHealthcare Community Plan lapses for more than 60 days, the Account will be moved to terminated status. The Participant will retain the ability to redeem any points earned prior to their account being terminated.

SPECIFIC MERCHANDISE REWARD TERMS AND CONDITIONS INFORMATION

Please review the terms and conditions applicable to specific rewards before redeeming. Terms and conditions imposed by participating merchants may apply.

POINT REDEMPTION AND MERCHANDISE REWARD INFORMATION

  1. Members can redeem points for Merchandise Rewards found in the online catalog associated with the Program.
  2. All rewards are subject to availability. Certain restrictions apply to rewards. Terms and conditions of each reward are set forth in Program communications and/or on the certificates and gift cards. Merchants participating in the Program are subject to change.
  3. Additional terms and conditions may apply when redeeming your points for rewards. You understand and agree to abide by any additional terms and conditions which apply. Rewards will not be replaced or reissued, nor points re-credited to an Account if the reward is lost, stolen, or otherwise destroyed. The Company, its affiliates and contractors will not be responsible for rewards or correspondence which are lost or delayed by the U.S. Mail or by another delivery service.
  4. Merchandise can only be shipped within the continental United States, Hawaii, Alaska, Puerto Rico and Canada unless otherwise noted. Your items may be shipped from different locations and arrive at different times.
  5. Items offered in connection with the Program are subject to change without prior notice.
  6. Should a product be discontinued or withdrawn, the Company may substitute a product of equivalent value, if available, or cancel your order and make an appropriate refund.
  7. Accidental typographical errors sometimes occur in merchandise descriptions and product values. In this event, the Company reserves the right to notify the Account Owner of the correct description or value before merchandise is shipped.
  8. The Company cannot accept responsibility for delays or inability to fulfill for any cause beyond its control such as fire, explosion, action of the elements, strikes or other differences with workers, restrictions or restraints imposed by law, rules or other order of public authority; or interruption of transportation.

LIMITATION OF LIABILITY FOR GOODS AND SERVICES ARRANGED THROUGH THE PROGRAM AND THIS SITE:

ANY AND ALL CLAIMS REGARDING ANY FAILURE OR BREACH WITH RESPECT TO GOODS AND SERVICES OFFERED THROUGH THE SITE ARE LIMITED TO CLAIMS AGAINST ANY AND ALL SUCH PROVIDERS OF GOODS AND SERVICES. UNITED HEREBY DISCLAIMS ANY LIABILITY, WHETHER BASED ON CONTRACT, TORT, STRICT LIABILITY OR OTHERWISE, INCLUDING WITHOUT LIMITATION LIABILITY FOR ANY DIRECT, PUNITIVE, SPECIAL, CONSEQUENTIAL, INCIDENTAL OR INDIRECT DAMAGES, IN CONNECTION WITH THE GOODS OR SERVICES PROVIDED BY ANY CARRIER OR OTHER SUPPLIER THROUGH THE SITE, INCLUDING WITHOUT LIMITATION LIABILITY FOR ANY ACT, ERROR, OMISSION, INJURY, LOSS, ACCIDENT, DELAY OR IRREGULARITY WHICH MAY BE INCURRED THROUGH THE FAULT, NEGLIGENT OR OTHERWISE, OF SUCH CARRIER OR SUPPLIER AND YOU HEREBY EXONERATE UNITED FROM ANY LIABILITY WITH RESPECT TO THE SAME.
THE MERCHANDISE PROVIDED UNDER THIS PROGRAM IS PROVIDED "AS IS" WITHOUT WARRANTY OF ANY KIND, EITHER EXPRESSED OR IMPLIED, INCLUDING, BUT NOT LIMITED TO, THE IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, OR NON-INFRINGEMENT. THESE WARRANTIES ARE HEREBY EXCLUDED TO THE FULLEST EXTENT PERMISSIBLE BY LAW. UNDER NO CIRCUMSTANCES SHALL THE COMPANY BE LIABLE FOR ANY DIRECT OR INDIRECT, SPECIAL, INCIDENTAL OR CONSEQUENTIAL DAMAGES THAT MAY ARISE FROM YOUR USE OF, OR INABILITY TO USE, THE MERCHANDISE.

GENERAL

1.Fraud or abuse relating to the accrual of points or redemption of rewards may result in forfeiture of accrued points as well as cancellation of your Program Account.
2.All questions or disputes regarding eligibility for the Program, eligibility of points, or redemptions of rewards will be resolved by the Company at its sole discretion.
3.The determination of tax liability for any federal, state or local taxes (as may be applicable) arising out of the accrual or conversion of points or redemption of rewards in the Program shall be your sole responsibility.
4.We are not responsible for typographical errors and/or photographic errors and/or omissions in these Terms and Conditions.
5.These Rules & Restrictions are governed by and construed under the laws (excluding conflicts of law provisions) of Minnesota.

MEDICARE

Plans are insured through UnitedHealthcare Insurance Company or one of its affiliated companies, a Medicare Advantage organization with a Medicare contract and a contract with the State Medicaid Program. Enrollment in the plan depends on the plan’s contract renewal with Medicare.

You must continue to pay your Medicare Part B premium, if not otherwise paid for under Medicaid or by another third party.

The products and services described in UnitedHealthcare Community Rewards™ are neither offered nor guaranteed under our contract with the Medicare program. In addition, they are not subject to the Medicare appeals process. Any disputes regarding these products and services may be subject to the UnitedHealthcare® Dual CompleteTM (HMO SNP) or UnitedHealthcare® Dual CompleteTM ONE (HMO SNP)grievance processes.
